Obtain Verification of Certification request forms
to obtain verification that a perspective occupational
therapy hire is either OTR or COTA certified and in
good standing.
Review the Disciplinary Action Information Exchange
Network (DAIEN) for a current listing of NBCOT® as well
as state regulatory board actions. Find out about NBCOT's
disciplinary action program and how to file a complaint
against a certified practitioner who may be in violation
of NBCOT's Candidate/Certificant Code of Conduct.
