Please contact NBCOT to confirm the status of your renewal by email (recert@nbcot.org) or phone (301-990-7979). Be prepared to provide details concerning the method in which you renewed as well as how you paid for your renewal.

Verification of certification letter requests can now be submitted online via the Verification of Certification portal. Online requests require that payment be made with a Visa or Mastercard, and will be processed within 10-15 business days.

You may fax the completed form with Visa or MasterCard information to our secure fax line at (301) 869-8492 or mail the form with check/money order. Paper requests will be processed within 30-45 business days.

Official score requests cannot be made online at this time.  Please print out and complete an Official Score Transfer Request form. You may fax the completed form with Visa or MasterCard information to our secure fax line at (301) 869-8492 or mail the form with check/money order. Score transfer requests will be processed within 10-15 business days.

Please contact your state board directly to determine what form they specifically require for licensure purposes.

The Online Renewal Application is set up as primarily a one-time use tool when you are due to renew.  You may keep us apprised of changes in your address, email address and phone numbers by emailing your previous and current information to changes@nbcot.org.  If we can be of further assistance, please contact us during normal business hours EST at 301-990-7979.

Please complete a Name Change Request form, submit the two forms of documentation noted on the form. 

If you are also completing a certification renewal application, please use the hardcopy renewal application and follow the steps for completing a name change on the front of the renewal application.  You will also need to submit two forms of documentation that reflect your new name:

  1. A government-issued document (driver's license, state-issued ID, or passport) AND
  2. The marriage certificate, divorce decree, or court order that shows the legal name change.

***Please note that sometimes driver’s licenses come through a bit dark when sent via fax, so if you are able to lighten the copy before faxing, that may be helpful.
