Applying for OTED

Applicants are required to complete the OTED application and provide documentation demonstrating compliance with the OTED criteria.

All documentation received must be submitted by the primary source, which means that the documents are sent to NBCOT directly from the respective agency. An example is the official transcript must be sent directly from the registrar’s office of the school/university, in a sealed envelope with the official seal or stamp of the school. Documents that are not sent to NBCOT from the primary source will not be accepted and may delay the processing of the OTED application.

8 Steps to complete the OTED Application

1. Determine if you are CATEGORY A or CATEGORY B applicant (Click on OTED Criteria under OTED Navigation for more information)

Category A – proceed to step #3

2. Category B applicants ONLY – Complete and submit a Determination of Masters Equivalence Content Evaluation Form

Course descriptions must be accompanied with the completed Determination of Masters Equivalence Content Evaluation Form. If the course descriptions are not in English, an official translation or an independent translation certified as authentic by your school is required.

3. Complete the OTED Application

  • The appropriate fees must accompany the completed application.
  • A signed passport-size photo must be included in the application.
  • If applicable, name change information and supporting documentation must be included.
  • If applicable, proof of power of attorney must be included.

4. Take and pass English Language Proficiency Exams (see requirements here)

Scores must be sent directly to NBCOT from Educational Testing Services (ETS)

ETS will need NBCOT’s institution code to process your request: Code 9953

5. Request transcript(s) from your OT school (and other related education)

Your college/university must submit your official transcript in occupational therapy directly to NBCOT. If you are combining education for review under Category B, you must submit transcripts for all related education listed on the Determination of Masters Equivalence Content Evaluation Form. If the original document is not received in English, a copy will be returned to you for an official translation or an independent translation certified as authentic by your school.

6. Request completion of the Program Director Form.

Complete the Applicant section of the form, and forward the form to the program director of your OT program. The completed form must be sent directly to NBCOT by the Program Director.

7. Request completion of the Verification of Academic Credential Form.

Please note: If the applicant’s transcript contains the information that is being requested on this form, the form does not have to be submitted. Otherwise, complete the Applicant Section of the form, and forward the form to the registrar of your OT school/program. The completed form must be sent directly to NBCOT by the registrar.

8. Request completion of the Verification of OT License, Registration, Certification, or Other form of Official Government Recognition Form, and temporary permit history (if available)

Complete the Applicant Section of the form, and forward the form to all government authorities that provided you with a license, registration or certification. Makes as many copies as necessary.

If a temporary permit history is available, it must also be verified by the government entity.

The form and temporary permit history must be sent directly to NBCOT from the government entity.
