Paper Exam Application

First-time exam candidates may submit their exam application using the downloadable PDF form found at the bottom of this page. 

  • Step 1: Read the instructions outlined in the Certification Exam Handbook (PDF).
  • Step 2: Complete all applicable sections of the application.  A signature is required on the Attestation Statement. Make a photocopy of the completed application for your records.
  • Step 3: Mail the application with payment to:

NBCOT recommends that the application be sent via traceable mail (e.g., certified). If a working email address and password were provided in the application, the candidate can use them to check the status of the application online at MyNBCOT, once the application has been processed.
