• Name/Address Change
  Keep your certification record current by notifying NBCOT^® of name or address changes. Name change documents must be notarized.
• Request for Duplicate Certificate
  Order a replacement for a lost certificate.
• Fax Verification Request Form
  To be used by employment agencies only! Fax verification is used to check certificant status only.  This is not the Official Verification Request Form for licensure to state boards.
